Care Coordination Counselor
Seeking a Care Coordination Counselor for a major Philadelphia Hospital for a temp to permanent position. Pay rate, depending on experience, is approximately $25/hr.

 
Job Description:

The Care Coordination/Counselor will provide clinical guidance to the following activities: 
• Serving as a resource to families and providers to facilitate communication and coordination across multiple clinical services 
• Providing educational resources to families and providers to support partnership and care coordination 
• Developing and providing tools to support family and provider access to clinical information • Partnering to build a network of key contacts/experts across the network to support care coordination activities 
• Coordination of scheduling of services for in-patients either while in house or upon discharge • Shared clinical oversight of the Department of Pediatrics Central referring physician line to facilitate communication and coordination of care Job Responsibilities:
• Educate families re: the concept of care coordination and the medical home • Help families and providers identify and agree upon medical home/key coordinators for care as well as mechanisms to communicate/share care plans 
• Refer families to other programs/providers as needed • Offer families tools - “Care Coordination for Families”
• Offer providers tools -“Care Coordination for Providers” • Identify approaches that will be most culturally effective fo specific families
• Refer families to peer to peer support, family support groups and other family resources
• Along with other internal experts, provide care coordination mentorship/support to divisional care coordination specialists/key contacts through monthly/quarterly forums and other mechanisms • Run monthly forums for families in partnerships with family leaders 
• Develop and maintain relationships with: Complex Scheduling/Case Management/Social Work, Primary Care, Family Advisory Committee, Cultural Effectiveness, internal care coordination, medical home programs, family resource centers, financial counseling, community resources, etc. • Work closely with international relations department 
• Oversee inpatient complex scheduling services
• Provide shared clinical oversight to the Dept. of Pediatrics central referring physician line to facilitate communication and coordination of care
• Partner with Access Nurse Advisor and Access Center to coordinate scheduling and provide back up support to this role as needed Experience RN, BSN preferred. 3 years experience, preferably in cardiac acute care environment
